House of Welcome Adult Day Program

Our Approach 

Using a person-centered approach, House of Welcome’s (HOW) Day Program offers structured, therapeutic, small-group activities in a comfortable, home-like setting. Creative arts therapies are a unique feature of our programming. Participants enjoy meaningful activities and friendships, while caregivers benefit from time on their own. 

At House of Welcome, we understand that caregivers play an integral role in the care journey. We offer a comprehensive caregiver support program designed to provide holistic assistance to those who are caring for a loved one experiencing memory loss.

We understand that caregivers need care too, and our program is tailored to address each family’s unique needs. We can provide access to respite care, resources, and one-on-one support to caregivers. To learn more about our Caregiver Support Program, click here.

Sessions & Rates

Families can choose from morning, afternoon or or full-day options. Participants must attend a minimum of 2 sessions per week. To secure a spot, a 1-month payment in advance is required.

  • Morning: 9 a.m. to 12:30 p.m.—$100/day; includes lunch
  • Afternoon: 1 to 5 p.m.—$100/day

Full Day: 9 a.m. to 5 p.m.—$150/day; includes lunch

How to Enroll in the Adult Day Program

To enroll in the House of Welcome (HOW) Adult Day Program, please follow the steps below. Once we receive the application forms and the medical report from your primary care doctor, we will schedule an interview with our Clinical Manager/Caregiver Specialist.

Enrollment Forms

The two enrollment forms are fillable PDFs. You can download them to your computer or print them and fill them out by hand. To save them to your computer, please add your family member’s name to the file name when you save it (for example, Demographics & Health History – Pat Jones). When they are complete, please email both files to HOW@nssc.org.

If you fill out the forms by hand, please print legibly. Forms may be mailed or dropped off at HOW, 1779 Winnetka Road, Northfield, IL, 60093, during business hours.

  1. Complete the Demographics & Health History form.
  2. Fill out the Personal History application.
  3. Download the HOW Medical Report and ask your family member’s primary care doctor to fax it to HOW at 847.242.6275.
